Recording session log sheets

I'm looking for some recording session log sheets. Where you log your levels, EQ, etc. during a session.

I asked GravityJim, but his equipment log does all that automatically so he just loads the file, and everything goes back to their last positions...

So if anybody still had a sample still around, or where I can find something like that, please let me know.

we used to use allen and heath board sheets a lot. i'm sure they might have them online somewhere.
